PPE Information

Use the form below to record what Personal Protective Equipment (PPE) you are missing, and/or schedule a time to come out to the OEM to get fit tested for your mask, and pick up your PPE.

*Note: The Camelbak, voice amplifier and voice amplifier bracket are no longer available/issued.

If you need a suit, check the box that corresponds with your height. If you do not need a suit, check the "no suit needed" box.





If you need boots, check the box with your proper shoe size. If you do not need boots, check the "no boots needed" box.




If you need a mask, you must be fit tested before it can be issued. Choose a date and time period you are available. You will be contacted with an exact time for your appointment.
